Mazars Death Review

I was the family representative for Connor Sparrowhawk’s family on the review commissioned by David Nicholson, shortly before he retired as CEO of NHS England. The review, conducted by Mazars, looked at the deaths of learning disabled people, and those using mental health services at Southern Health NHS Foundation Trust between April 2011 and March 2015.

The review sought to establish the extent of unexpected deaths in mental health and learning disability services provided by the Trust, and to identify any themes, patterns or issues that may need further investigation. It found that only 1% of the deaths of learning disabled people were investigated.
